print them out and send them with your photograph to the address below. Forms may be reproduced on computer and should
be typed. A link to show you an example of how your fact sheet should look is also provided below.
Contestants will appear on stage in the opposite order of paperwork received. Talent
requests will be granted on a first come, first serve basis, so get your paperwork in early!
Once your application materials are received more information will be sent to you.
